Effective date: October 17, 2026
These Terms and Conditions (“Terms”) are an agreement between you and LightWeight MD, PLLC (“LightWeight MD,” “we,” “us”). They apply when you use lightweightmd.com, our patient portal, or our medical services. By creating an account or starting a visit, you agree to these Terms. If you do not agree, do not use our services.
These documents are also part of these Terms: our Privacy Policy, our Notice of Privacy Practices, our Refund and Cancellation Policy, and the Telehealth and Treatment Informed Consent you sign before your first visit.
Please read Section 7 (subscription and automatic renewal) and Section 17 (disputes) carefully.
LightWeight MD, PLLC is a South Carolina medical practice owned and operated by Henry Igdal, M.D., a physician licensed in South Carolina. We provide medical weight-management care by telehealth to adults located in South Carolina.
Our care includes a medical evaluation, a live video visit with a physician, prescriptions when medically appropriate, and ongoing follow-up and dose adjustments. Prescriptions are filled by Pharmcore, Inc. d/b/a Hallandale Pharmacy Compounding (“the pharmacy”), which holds an active South Carolina Non-Resident Pharmacy Permit.
The general information on our website is for education only and is not medical advice. Medical advice comes only from your physician as part of your care.
To use our medical services, you must:
Meeting these requirements lets you sign up. It does not mean you qualify for treatment; your physician decides that.
We do not provide emergency care. If you have a medical emergency, call 911 or go to the nearest emergency room. Do not use our portal, email, or phone line for urgent symptoms.
Your care is delivered by telehealth, including a live video visit on a HIPAA-compliant platform. Telehealth has limits: your physician cannot physically examine you, and technical problems can interrupt a visit. Your physician may decide telehealth is not right for you and recommend in-person care. By starting a visit, you consent to receive care by telehealth.
We respond to portal and email messages within 3 business days. If you have a symptom that cannot wait that long, contact your primary care provider or urgent care, or call 911 in an emergency.
We do not replace your primary care provider. You can request free copies of your records at any time to share with your primary care provider or other doctors.
Your physician-patient relationship with LightWeight MD begins when your physician reviews your information and completes your video visit. Completing the questionnaire, creating an account, or choosing a plan does not create that relationship and does not guarantee a prescription.
Your physician uses independent clinical judgment. Based on your health and safety, your physician may:
Your subscription never overrides these decisions. Results vary, and we do not promise any specific amount of weight loss.
The medications we prescribe may include compounded semaglutide or compounded tirzepatide, prepared by a licensed compounding pharmacy for you individually.
Compounded medications are not FDA-approved. FDA does not review them for safety, effectiveness, or quality before they are dispensed. They are not generic versions of Ozempic, Wegovy, Mounjaro, or Zepbound, and they are not the same as those products.
Your dose increases gradually on a set schedule. Your physician may hold or lower your dose if you have side effects.
FDA-approved versions of these medications, such as Wegovy and Zepbound, are available by prescription. They usually cost more and are not currently offered through LightWeight MD. You can ask your physician about them at any time, and you may choose to get an FDA-approved medication elsewhere.
Federal or state rules can change whether compounded medications are available. If we can no longer prescribe your medication, or the pharmacy can no longer supply it, we will tell you promptly, stop future charges, and help you move to another treatment.
Your physician sends prescriptions electronically to the pharmacy, which prepares and ships your medication. All of our prescriptions are filled by this pharmacy; we do not send prescriptions to other pharmacies.
If your medication arrives damaged or incorrect, or never arrives, our Refund and Cancellation Policy explains your options.
Your medication is for your personal use only. Do not share, sell, or give it to anyone else. Dispose of needles in a sharps container.
Your plan renews automatically every month and charges your card until you cancel. You can cancel anytime (Section 8).
| Medication | First month | Each month after, below the top dose | At the top dose |
|---|---|---|---|
| Compounded semaglutide | $149 | $200 | $230 (2.5 mg) |
| Compounded tirzepatide | $275 | $300 | $340 (15 mg) |
Your monthly price includes your medication, shipping, physician visits, follow-up, and dose adjustments. There is no membership fee.
When we charge. We do not charge you when you sign up. We charge your first month only if your physician approves your prescription at the end of your visit. If you are not approved, you are not charged. Each renewal is charged on the same day of the month as your first charge (or the last day of the month, in months that are shorter). By subscribing, you authorize us to charge your card on file, through our payment processor Stripe, for these amounts.
Price change at the top dose. If your physician moves you to the top dose, your monthly price changes to the amount shown above. We will tell you the new price before the first charge at that price.
Other price changes. We may change prices for future months. We will email you at least 30 days before a new price applies, and you may cancel before it takes effect.
Failed payments. If a charge fails, we may pause shipments until payment goes through. Your physician remains available for medical questions while shipments are paused.
You may cancel anytime in your patient portal or by emailing billing@lightweightmd.com. Cancellation takes effect at the end of your current billing cycle, and there is no cancellation fee.
We do not refund partial months. Our Refund and Cancellation Policy explains when you can get a refund, such as when medication has not yet shipped or arrives damaged.
If you have a billing problem, please contact us first; we respond within 5 business days. If you file a chargeback without contacting us, we may pause shipments until the dispute is resolved. Your physician remains available for medical questions during that time.
You agree to:
False health information can be dangerous and may lead us to stop treatment.
Keep your login private. You are responsible for activity on your account, so tell us right away if you think someone else has used it.
You agree to receive notices, receipts, and other communications from us electronically, by email, text message, or in the patient portal. Electronic notices have the same effect as paper ones. Our Privacy Policy explains our text-message practices and how to opt out.
You may stop care at any time by canceling your subscription.
We may end our relationship with you, or suspend your account, if you break these Terms, give false information, are abusive toward our staff, or if continuing care is not medically appropriate. Except when safety requires faster action, we will give you at least 30 days’ written notice. During that time we will remain available for urgent medical needs and help you transfer your care and records.
Terms that by their nature should continue after our relationship ends, such as payment obligations, disclaimers, and limits of liability, remain in effect.
We own the content on lightweightmd.com, including text, graphics, and logos, or have permission to use it. You may view and print it for your personal, non-commercial use. You may not copy, change, or distribute it without our written permission.
Ozempic, Wegovy, Mounjaro, and Zepbound are trademarks of their owners. We name them only to identify the FDA-approved products. Their owners do not sponsor or endorse LightWeight MD.
You may not:
Our website and patient portal are provided “as is” and “as available.” To the fullest extent the law allows, we disclaim implied warranties for the website and portal, including merchantability, fitness for a particular purpose, and non-infringement. We do not promise the website will always be available or free of errors.
This disclaimer does not apply to the medical care your physician provides. That care is held to the professional standard of care that South Carolina law requires.
To the fullest extent the law allows, LightWeight MD is not liable for indirect, incidental, special, consequential, or punitive damages arising from your use of the website or portal. Our total liability for any claim about the website, portal, or billing is limited to the amount you paid us in the 12 months before the claim.
These limits do not apply to claims about the medical care we provide, to our gross negligence or willful misconduct, or to any liability that South Carolina law does not allow us to limit.
If you use the service for another person, use someone else’s identity, or knowingly give false information, you agree to cover the reasonable costs, including legal fees, that LightWeight MD incurs from claims caused by that conduct.
Contact us first. If you have a concern, email info@lightweightmd.com. We will try to resolve it informally within 30 days.
Governing law. South Carolina law governs these Terms, without regard to conflict-of-law rules.
Where disputes are heard. Any dispute not resolved informally will be decided in the state or federal courts located in Greenville County, South Carolina, and you and we consent to those courts. You may also bring an individual claim in magistrate’s court if it qualifies.
Complaints to regulators. Nothing in these Terms limits your right to file a complaint with the South Carolina Board of Medical Examiners, the South Carolina Board of Pharmacy, or any other government agency.
We may update these Terms. For material changes, we will post the new version with a new effective date and email active patients at least 30 days before the change takes effect. If you do not agree, you may cancel before then. If you keep using our services after that date, the updated Terms apply.
Changes do not apply to disputes that arose before the change.
